I, the parent, hereby give consent for my child to participate in activities organized by Jaffari Tabligh Board/Jaffari Children's Tabligh at the premises of Jaffari Community Centre at 9000 Bathurst Street, Thornhill, Ontario. I accept all responsibility for my child’s safety during the duration of the camp from August 12-16, 2024 and release the camp organizers of any responsibility and liability of any unexpected injury or claim resulting from this participation. My child and I also agree to abide by all rules and regulations set out by the Camp Organizers. Additionally, I, the undersigned, hereby authorize the Jaffari Tabligh Committee/Jaffari Children's Tabligh to utilize the camper’s name and/or photographic or verbal representation by any media format in the promotion of the programs of Jaffari Tabligh Committee/Jaffari Children's Tabligh.
We are in need of volunteers. Would you be willing to volunteer your time in assisting us?
- Yes, I would love to help in preparing and am able to also volunteer at the premises during the camp.
- Yes, I can help with prepararing materials but am not able to participate during the camp.
- No. I cannot help in any capacity
Payment
The fee for each camper is $150 for the duration of the week.
